How do I answer NAVLE next-best-step questions?

Prepared 23 October 2026 · VetQ

A next-best-step question asks you to sequence decisions, not simply recognise a disease. Read what is already known, what remains uncertain and whether the patient is stable. A useful investigation or treatment may still be the wrong action at that moment.

Identify the stage of the case

Are you at initial triage, choosing a diagnostic test, interpreting a result or selecting definitive management? Do not repeat an investigation already completed unless the stem gives a reason. Keep immediate priorities distinct from later actions.

Compare actions by what they achieve

For each plausible option, ask what information or benefit it provides now. Consider whether an action addresses the main problem, changes management or introduces an avoidable delay. Use the actual case evidence rather than applying a memorised sequence blindly.

Explain why the alternative comes later

When reviewing, write not only why the chosen action fits but also when the other plausible action would become appropriate. This turns a single question into a lesson about sequencing that can transfer to a new patient.

Use a decision ladder

For a practice case, write the current problem, the immediate risk, the information already available and the remaining uncertainty. Then place the plausible options in a reasoned order. Explain why one belongs now, another might belong later and another would not help the stated problem. Compare that ladder with the reviewed explanation. If you chose the right action for the wrong reason, keep the case open for review. Changing the wording from “initial action” to “definitive management” is a useful teaching variation. It shows how the same patient can support different answers when the requested decision changes.

Check your understanding

You identify the likely diagnosis, but two options are a stabilising action and a definitive procedure. What must you check before choosing?

Read the answer and reasoning

Check the patient’s stability, the exact task and what the stem has already established. The diagnosis alone does not decide the next step. The best action depends on the clinical stage and immediate priorities.
